Administration of medicines in school – Information for Parents
Many pupils will, at some time, need to take medication in school.
If your child needs to take any medication at school then the appropriate
forms must be completed before school staff can administer medicines,
or allow children to administer their own medication.
Please download the forms required from this page and ensure that you
complete the form for either prescribed or non-prescribed medication.
Form 2a/b is for prescribed medicines that you want
school staff to administer

Form 3 is for medication that you give consent for your
child to administer themselves

Form 5 is for non-prescribed medication e.g. paracetemol
